Vitamin D: Why Almost Half of India Is Running Low, and What To Actually Do About It

Here’s something that surprises most people the first time they hear it.

India gets more sunshine in a single month than most of Europe gets in a year, yet a huge chunk of the country is deficient in the one vitamin that sunlight is supposed to give us for free.

Doctors call it the sunshine vitamin. The irony writes itself.

A large nationwide study by Metropolis Healthcare, which tested samples across the country, found that 46.5 percent of people were deficient and another 26 percent were insufficient. Add those two numbers together and you get roughly 7 out of 10 Indians walking around without enough vitamin D in their blood, even in a country where sunlight is rarely in short supply.

A separate systematic review covering South Asia put India’s deficiency rate at 67 percent among adults, and a Delhi-based study on the National Capital Region found 71 percent of urban adults severely deficient, compared to 20 percent in rural areas of the same region.

The gap between city and village life shows up clearly in the data, and it points straight at how modern Indians actually live: indoors, in offices, behind car windows, under sunscreen, and rarely outside between 10 am and 3 pm when the body can actually make the vitamin.

This article breaks down what vitamin D actually does, why so many Indians are short on it despite the sun, how it connects to muscle and hormone health for men specifically, and what the research says about fixing it through sunlight, food and supplements.

What Vitamin D Actually Is, and Why Calling It a “Vitamin” Undersells It

Technically speaking, vitamin D behaves less like a vitamin and more like a hormone.

Your skin makes it when ultraviolet B rays from sunlight hit a compound called 7-dehydrocholesterol, converting it first to a precursor and then, through the liver and kidneys, into the active form your body uses.

That active form then travels through the bloodstream and switches on vitamin D receptors in bone, muscle, immune cells and dozens of other tissues.

This is why a shortage of it doesn’t just affect your bones. It affects calcium absorption, muscle contraction, immune signalling and, as newer research is starting to show, hormone metabolism as well.

Most people still think of vitamin D purely as a bone-health nutrient tied to calcium. That picture is decades out of date.

Why a Tropical Country Still Falls Short

If sunlight were the whole story, India should have close to zero deficiency. The research points to a mix of reasons why that isn’t the case.

Urban Indians spend most daylight hours indoors, in offices, cars and homes with tinted glass, which blocks the UVB wavelength needed for synthesis.

Skin pigmentation also plays a role.

Melanin, the pigment that gives Indian skin its tone, absorbs UV rays and slows down vitamin D production, meaning darker skin genuinely needs more sun time than lighter skin to make the same amount.

Pollution in major cities scatters and absorbs UVB before it ever reaches street level.

And dietary habits don’t help either, since very few traditional Indian foods are naturally rich in vitamin D, and fortification of foods like milk is still far from universal.

A national roadmap report published through ICRIER puts regional deficiency between 9.4 percent in parts of North India and as high as 38.8 percent in East India, showing this isn’t a uniform problem but one shaped heavily by lifestyle, latitude and local diet.

Interestingly, the same body of research shows some encouraging movement too.

Deficiency levels nationally are estimated to have dropped from around 51 percent in 2019-20 to about 43 percent in 2023-24, likely a result of growing awareness and wider supplement use following the pandemic years, when vitamin D testing became far more common.

Signs Your Body Might Be Waving a Red Flag

Vitamin D deficiency is sneaky.

A large number of people carry it for years with no symptoms sharp enough to notice, which is exactly why doctors recommend a blood test rather than guesswork. But when symptoms do show up, the pattern research has documented includes persistent tiredness that doesn’t improve with sleep, dull aching in the bones or lower back, muscle weakness or cramping, getting sick more often than people around you, slower healing of cuts and wounds, and low mood that some studies link with depressive symptoms, though the evidence there is still developing rather than settled.

None of these symptoms are exclusive to vitamin D deficiency on their own.

Fatigue alone could mean twenty different things. That’s precisely why a 25-hydroxyvitamin D blood test, not a symptom checklist, is the only reliable way to know where you actually stand.

Vitamin D and the Modern Indian Man: The Testosterone Question

Vitamin D receptors sit directly on muscle tissue and on the Leydig cells in the testes that produce testosterone, which is the biological basis for why researchers started studying the connection in the first place.

Some trials back this up. A twelve-week resistance training study found that correcting vitamin D deficiency alongside diet changes supported hormone balance in strength-training men.

Animal and cell studies published in the Journal of Cachexia, Sarcopenia and Muscle in 2025 found that testosterone actually amplifies how vitamin D affects muscle strength, suggesting the two work together rather than one simply causing the other.

But here’s where honesty matters more than hype. A well-designed randomised controlled trial on 180 young Indian men with clear vitamin D deficiency found that six months of cholecalciferol and calcium supplementation produced no significant change in grip strength or testosterone levels.

The takeaway from the wider body of research is fairly consistent: correcting a genuine deficiency may support the systems that regulate testosterone and muscle function, but popping vitamin D pills when your levels are already normal is unlikely to push your testosterone higher.

Vitamin D is not a testosterone booster in the way protein powder ads sometimes imply. It’s closer to a foundation you need in place before other things can work properly, not a shortcut on top of that foundation.

How Much Vitamin D Do You Actually Need

Global nutrition bodies, including the US Institute of Medicine, set the Recommended Dietary Allowance at 600 IU (15 mcg) per day for most adults aged 18 to 70, rising to 800 IU (20 mcg) for adults above 70.

These numbers assume minimal sun exposure, which is a fair assumption for most working professionals today. The tolerable upper limit, meaning the ceiling you shouldn’t cross without medical supervision, sits at 4,000 IU per day for adults.

For someone already diagnosed as deficient through a blood test, doctors in India commonly prescribe much higher short-term doses, sometimes 60,000 IU once a week for eight weeks, followed by a lower maintenance dose.

This is a supervised medical correction, not a number to self-prescribe from a pharmacy shelf. If your blood report shows a level below 20 ng/mL, that conversation belongs with a doctor, not a fitness influencer.

Sunlight, Food or Supplements: Where Should You Actually Get It

Sunlight, done right

Multiple studies converge on a similar window: exposing your face, arms and legs to direct sunlight between 10 am and 3 pm, for roughly 15 to 30 minutes, two to three times a week, is enough for most people with Indian skin tones to maintain healthy levels.

Morning walks at 7 am, however pleasant, contribute almost nothing to vitamin D synthesis, because the UVB wavelength needed is too weak that early. The sun has to be reasonably high in the sky, which is why the unglamorous midday hours actually matter more than sunrise walks for this particular purpose.

Food, a smaller but real contributor

Very few whole foods carry meaningful vitamin D naturally. Fatty fish like salmon and mackerel top the list, followed by egg yolks, mushrooms exposed to UV light, beef liver, and fortified products such as certain milks and cereals.

For a largely vegetarian population, this is genuinely one of the harder micronutrient gaps to close through diet alone, which is part of why supplementation is so widely recommended in the Indian context specifically.

Supplements, and the D2 versus D3 question

When food and sunlight fall short, cholecalciferol, known as vitamin D3, is the form most research and most Indian physicians recommend over ergocalciferol, or D2, because D3 raises and sustains blood levels more effectively. It’s available as weekly sachets, daily capsules and drops, and is inexpensive relative to most other supplements on the market.

Too Much of a Good Thing: Toxicity Is Rare But Real

Unlike water-soluble vitamins that your body simply flushes out, vitamin D is fat-soluble and can build up in tissue over time. Toxicity typically only shows up at intakes above 10,000 IU a day sustained over a long period, and it’s almost always the result of self-prescribed megadoses rather than food or moderate sun exposure.

Symptoms include nausea, weakness, and in serious cases, dangerously high calcium levels in the blood that can affect the heart and kidneys.

This is precisely why doctors don’t hand out 60,000 IU weekly doses indefinitely. It’s a correction protocol with a defined endpoint, not a permanent routine.

Getting Tested Is the Only Real Starting Point

Everything in this article changes depending on one number: your 25-hydroxyvitamin D blood level. A level below 20 ng/mL counts as deficient, 21 to 29 ng/mL is considered insufficient, and 30 ng/mL and above is generally taken as sufficient.

This single blood test costs relatively little in most Indian cities and removes the guesswork entirely.

Given how common deficiency is, and how symptom-free it often stays until it isn’t, an annual test is a reasonable habit for most adults, not an overreaction.

The Bottom Line

Vitamin D deficiency in India isn’t a marketing problem invented to sell supplements. The numbers from Metropolis Healthcare, ICRIER and multiple peer-reviewed Indian studies all point the same direction: a majority of Indians, especially in cities, are running short on a nutrient the sun is handing out for free just outside their office windows. Fixing it isn’t complicated.

Fifteen to thirty minutes of real midday sun a few times a week, a diet that makes room for the handful of foods that actually carry vitamin D, and a supplement when a blood test says you need one.

What it does need is testing before assuming, and a doctor’s guidance before dosing high. The sunshine vitamin rewards consistency far more than it rewards a single dramatic fix.
